How we use your health information

We respect your privacy and keep your information confidential. This sheet explains why we collect your details and how we use them.

Scope

We collect and use your health information to maintain an accurate record of your care, ensuring you receive safe and high-quality treatment whenever needed.

It is also used for:

• Safety and wellbeing of you and others
• Improving health service funding and planning
• Strengthening service quality and management
• Public health statistics input
• Healthcare professional education and training
• Conducting authorised research

Privacy and Confidentiality

We value your privacy and are committed to protecting the confidentiality of your information. Your health information is securely stored to prevent unauthorised access.

During your consultation, your health practitioner will record all relevant details in your health record. Your information may be shared with others directly involved in your care, such as authorised agencies with your consent or as permitted by law.

You are not required to share your health information; however, withholding certain details may affect the quality of care you receive. If you have any questions or concerns, please speak with your health practitioner.

As your primary healthcare provider, the PHO will be notified of any casual visits you make to other providers if you are under eighteen years of age, or if you hold a Community Services Card or a High Use Health Card.

Information Accuracy and Quality

We are committed to maintaining your health information so that it is accurate, current, and relevant to your care. You have the right to access and request updates or corrections to the information we hold about you.

You may request to view or obtain a copy of your health information at any time. While you do not need to provide a reason for your request, proof of identity may be required. If a second copy of the same information is requested within a 12-month period, an administration fee may be charged.

If you believe any details are incorrect or incomplete, you can ask for them to be amended. Our staff will assist you with this process. Should your healthcare provider decide not to make the requested change, a note will be added to your record to indicate your request and the reason for the decision.

Greenlane Doctors are registered with ManageMyHealth, which enables patients to access certain parts of their health records online. Please contact our reception if you would like to sign up.

Complaints

Complaint services are available if you’re unsatisfied with how your health information is collected or used.
